could tension headache be caused by deficiency in magnesium ? How much should a female over 55 take calcium and magnesium and vit D per day ?

Vitamin D and calcium supplementation may also reduce headaches associated with premenstrual syndrome (PMS).

I dont think there is any relation between them, it depends upon sickness or disease type.

Stress is the main thing that causes head ache.

According to a 2003 telephone interview based study in the journal Archives of General Psychiatry, 40% of individuals who reported experiencing an episode of major depression also had a chronic painful condition, such as headache or backache. Moreover, participants who reported both depression and a chronic painful condition had, on average, depressive episodes that lasted six months longer than individuals who did not report any chronic pain. "Therefore," the authors of the study noted, "pain could also contribute to the prolongation of a depressive episode."

A lot of people don't get enough D these days because they aren't out in the sun all day, and when they are they're usually wearing sunscreen. It's a good idea to take lots of D.

Our body needs, every vitamin and protein in a good proportion. If we intake these vitamin as in form of pills, then in my opinion, it doesn't work well.
Try to take balanced diet and increase the consumption of milk. your body will then automatically meet the requirement of calcium and vitamins.
